Heart to Heart Hospice - - Responsibilities: Promote a customer-focused hospice presence in healthcare communities including assisted living facilities, nursing homes, home health, and hospitals; Coordinate care among healthcare professionals; Communicate weekly with patient and family to ensure needs are met and satisfaction; Identify problems or dissatisfaction and ensure quick resolution with clinical team; Coordinate and provide in-services or educational events for family members and facilities
