Heart to Heart Hospice - - Responsibilities: Provide direct patient care to patient under direction of the RN; Assist with safe transfers and ambulation per agency policy; Report/communicate changes and document pertinent information and care provided to patient; Practice infection control principles; Promote positive, supportive, respectful communication to patient/family and other employees
