Job Duties:

• Receiving and managing referrals directly from Adult Protective Services.
• Conducting in-depth assessments, identifying client needs, strengths, and self-directed goals.
• Applying trauma-informed care and client-centered practices.
• Performing required home visits and maintaining accurate and confidential records. Providing direct support and advocacy, particularly for older adult populations.
• Will work other work locations as needed.

• Must have a valid Texas Driver's License and comply with AP 2-2 policy.
• Requires a Bachelor's degree in a social or behavioral science, such as Sociology, Psychology, Social Work or Counseling
• Two years of professional experience in counseling or related social services work are required .
• A Master's degree in a social or behavioral science may be substituted for the experience requirement.
